Please contact your doctor if you have any medical condition to get the clearance to join dance classes as it involves complete body movement and physical workout.
The student who received this application (or where the student is below 18yr of age, the parent or guardian) hereby confirms that the student has been advised by a competent medical doctor that they are medically, mentally and physically fit to participate in the dance competition organized by Star Talent Foundation Inc / High School Dance Competition and that they have no physical or mental conditions that may affect their ability to do so.
It is expressly understood by the student and or the students’ parents or guardians or their school staff as the case may be (from now on referred to as student for the sake of convenience), that it is not compulsory to participate in the dance competition, shows events organized by High School Dance Competition. The student expressly confirms and states to Star Talent Foundation Inc / High School Dance Competition that they have voluntarily chosen to participate and or perform out of their own free will and not under any pressure from Star Talent Foundation Inc / High School Dance Competition, its representatives, instructors or anyone whatsoever to do so with Star Talent Foundation Inc / High School Dance Competition.
The student understands, acknowledges and accepts that participation in the dance shows/events organized by Star Talent Foundation Inc / High School Dance Competition is a strenuous physical activity and may involve the risk of accident, injury to the student’s person or property, or loss of life or harm of any nature to any student or student’s parent, guardian, relative or friend at or around the show venue the student expressly agrees and acknowledges that Star Talent Foundation Inc / High School Dance Competition shall not be responsible and or liable for the loss, injury, loss of life, harm or damage caused and the student shall further be liable to Star Talent Foundation Inc / High School Dance Competition for all loss and or damage suffered by Star Talent Foundation Inc / High School Dance Competition and or its representatives, instructors, employees, etc. virtue thereof.
Notwithstanding the above, in the event of any injury or other medical conditions that the student may experience during any of the classes or shows, the student authorizes Star Talent Foundation Inc / High School Dance Competition to administer or procure the administration of medical treatment as deemed fit by qualified medical personnel if they are not able to act on their own behalf and agrees not to make any claim against Star Talent Foundation Inc / High School Dance Competition and or such medical practitioners, was for negligence and or malpractice or otherwise. The student further agrees and acknowledges that they will be exclusively liable and responsible for all costs, expenses and claims resulting from such medical attention provided and shall reimburse Star Talent Foundation Inc / High School Dance Competition any sums expended by it in this regard.
The student shall further indemnify Star Talent Foundation Inc / High School Dance Competition regarding any claims, demands, losses or decrees, orders or awards that may be made against Star Talent Foundation Inc / High School Dance Competition and or its representative, tutors, instructors, employees, etc. in this regard. The student shall further indemnify Star Talent Foundation Inc / High School Dance Competition for all losses, damages and injury caused, resulting from or in any way attributed to any illness, accident, or injury of any of the members of Star Talent Foundation Inc / High School Dance Competition occasioned by any act or omission of the student.
Star Talent Foundation Inc / High School Dance Competition reserves the right to reject the participation of the student in any competition and or shows for reasons of inability, behaviour endangering the safety, the welfare of themselves and or others. No refund will be made to the student on account of such rejection or the student’s own inability to participate therein.
Star Talent Foundation Inc / High School Dance Competition programs and participating events will be shown or covered by the media. Your enrolment with Star Talent Foundation Inc / High School Dance Competition confirms that you have no objection to the same and will co-operate with Star Talent Foundation Inc / High School Dance Competition for all such publicity, television and media coverage and publishing of dance photographs, video on Star Talent Foundation Inc / High School Dance Competition website and some public websites like YouTube, Facebook or any other file sharing websites.

Agreeing for your child to enter a dance competition confirms that the student and the parent’s / guardian agree to the above terms and conditions.
